Kindergarten Registration
To protect the personally identifiable information (PII) of your child, please fill out these forms and return, as soon as possible, to The Searsport Elementary School. Once you have completed the forms:
1. Email the completed forms to [email protected]
2. Print out and mail the completed forms (note the envelope will require extra postage) to:

3. Drop off the forms at Searsport Elementary School between 8 AM and 2 PM Monday-Friday.

The Kindergarten Registration packet contains the following documents, please be sure to fill in all of the information:
- RSU #20 Emergency Form
- Maine Migrant Education Program Survey
- Home Language Survey
- Developmental History
- Student Health Questionnaire
- Proof of Residency Form
- RSU #20 Bus Information Sheet
You will also need to include the following:
- A copy of your child’s birth certificate.
- A copy of your child’s immunization record. The school nurse will review this with you at a later date.
- A proof of residency document as requested in “Proof of Residency” Form.
